  • 摘要(中) 一、中文摘要 四神聰穴位於頭頂由四個穴位組成,屬於經外奇穴,臨床針刺四神聰穴用來治療壓力性失眠,但是它的治療機轉並不十分了解。本篇論文將探討針刺四神聰穴與自主神經功能的關係,進而闡述針刺四神聰治療失眠的可能機轉。本研究採用本實驗室自行製作及撰寫之心電放大器及心率、血壓頻譜分析程式,並採目前最新頻譜分析之參數代表自主神經活性之指標。兩組各十名健康男性志願者,在下午二至四點平躺狀態下,接受針刺四神聰實驗。記錄針刺前後的心電與動脈壓訊號,進行自頻譜及交叉頻譜之轉換函數與相關函數分析。動脈壓及心率頻譜分析,可分成三個頻段:極低頻(0.00-0.04 Hz)、低頻(0.04-0.15 Hz)及高頻(0.15-0.4 Hz),三個頻帶加起來代表全頻。我們發現針刺四神聰穴能明顯增加心率高頻成份及顯著抑制心率低頻成份所佔的比重[低頻成份﹐(全頻成份-極低頻成份)×100%],並能抑制動脈壓的低頻成份,另外由動脈壓及心率的交叉頻譜分析之轉換函數,發現針刺四神聰穴後幅度(magnitude)有明顯增加之現象,目前學者廣泛接受心率頻譜分析中的高頻成份可代表副交感神經之活性,而心率頻譜分析中的低頻成份所佔的比重及動脈壓頻譜分析中的低頻成份可反應交感神經活性,而動脈壓及心率的轉換函數之幅度可代表感壓反射的靈敏度。由以上結果推論,針刺四神聰穴能興奮心臟迷走神經活性而抑制心血管交感神經活性,並增強感壓反射之靈敏度,這可解釋針刺四神聰穴可治療壓力性失眠。 關鍵詞:針刺、四神聰、自主神經系統、自頻譜分析、交叉頻譜分析、感壓反射
    摘要(英) 二、英文摘要 Sishencong is composed by four-points which are located on vertex of the head. Mild acupuncture stimulation of Sishencong has been used to treat tension insomnia. The underlying mechanisms, however, had not well been understood. The present study tried to find out whether Sishencong acupuncture has an effect on the autonomic nervous system (ANS). Acupuncture stimulation was manually applied at Sishencong points in 20 normal male volunteers, while lying in a quiet room during 2 to 4 P.M. Forty-min precordial ECG and arterial pressure before and after acupuncture were recorded non-invasively. Arterial pressure (APV) and heart rate variabilities (HRV) were subsequently divided into very-low frequency (VLF, 0.00-0.04 Hz), low-frequency (LF, 0.04-0.15 Hz) and high-frequency (HF, 0.15-0.4 Hz) powers in the frequency domain. VLF and LF and HF equal total power (TP). We found that manual acupuncture of Sishencong points enhanced HF but suppressed LF% [LF/(TP-VLF)*100%] of HRV. It also suppressed LF of APV. It is generally accepted that HF of HRV is predominantly modulated by the parasympathetic nerves whereas LF% of HRV and LF of APV are under the influence of the sympathetic nerves. We note that stimulation of Sishencong points enhances the magnitude of AP-HR transfer function. It revealed that the sensitivity of spontaneous barorecepor reflex was improved by acupuncture at Sishencong points. The results suggest that Sishencong acupuncture simultaneously enhance cardiac vagal activity and suppress cardiac and vascular sympathetic activities in humans. It also augments the sensitivity of baroreflex. The result can explain the therapeutic mechanism on tension insomnia by acupuncture at Sishencong points.
